IOP vs PHP: what actually separates the two

Both are outpatient. You sleep at home on either one. The difference is hours in the day, and that difference is what decides whether you can keep working while you are in treatment.

The short answer

Same bed at night. Different number of hours.

Partial hospitalization, ASAM level 2.5, is a day program. It runs most of the day on most weekdays, and it suits someone who needs a high number of structured clinical hours but does not need overnight supervision.

Intensive outpatient, ASAM level 2.1, runs fewer hours across the week, which is what leaves room for a job, school or caregiving. Shift delivers it in three to five hour blocks on weekdays, in a morning, afternoon or evening cohort.

Neither one is residential treatment, inpatient detox or acute psychiatric stabilization. Those need a building, and if that is the level you need we will say so at the screen and help you find it.

Side by side

The comparison, line by line.

ASAM level

Intensive outpatient is 2.1. Partial hospitalization is 2.5.

Hours in the day

Intensive outpatient runs fewer hours. Partial hospitalization runs most of the day on most weekdays.

Where you sleep

At home on both. Neither is residential care.

Work, school, caregiving

Intensive outpatient is built to leave room for them. Partial hospitalization usually does not.

Typical course

Eight to twelve weeks at the intensive outpatient level. Partial hospitalization intensity is usually shorter, three to six weeks, with a planned step down.

Authorization

Plans review the higher level of care more closely, and most want it approved before it begins.

What Shift is certified for

Outpatient (ASAM 1.0), intensive outpatient (ASAM 2.1) and ambulatory withdrawal management, under DHCS certification 191663AP. Hour expansion to partial hospitalization intensity is case by case and needs payer authorization.

Stepping between them

Down from 2.5 to 2.1 as someone stabilizes, up when they need more. Both moves are documented clinical decisions.

What neither one covers

Medical detox in a bed, residential treatment, acute psychiatric stabilization.

How the choice is made

A licensed clinician applies the ASAM Criteria at the assessment. The plan then reviews it.

Cost and insurance

We read your plan before you commit to anything.

Covered by many PPO plans. We check your benefits first.

Shift bills most major commercial plans on an out-of-network basis, and in-network credentialing is in process. We read your plan, tell you what it pays toward intensive outpatient care, and give you that number in writing before your first appointment.

Medi-Cal. We cannot bill Medi-Cal today, because we hold no county contract yet. If Medi-Cal is your coverage, tell us and we will point you to a county provider who can help now.

Level-of-care questions

The ones people ask first.

Is PHP a higher level of care than IOP?

Yes. Partial hospitalization sits one step above intensive outpatient on the ASAM continuum, at level 2.5 against 2.1, because of the number of structured clinical hours, not because of where you sleep.

Is IOP cheaper than PHP?

Fewer hours generally means fewer billed units, so the total is usually lower. What you personally owe still depends on your deductible, your coinsurance and your out-of-pocket maximum. The cost page walks through those.

Can I step down from PHP to IOP?

That is the normal path, and Shift is often the landing spot for someone stepping down out of a day program or a residential stay. See what the first weeks of outpatient care look like.

Which one do I need?

A licensed clinician answers that at the assessment, using the ASAM Criteria rather than a preference. How that decision gets made is here.

Is IOP the same as PHP?

No. They are two separate levels of care with separate criteria, separate hour expectations and, on most plans, separate authorization rules.

Find out which level fits.

A benefit check and a short clinical screen confirm whether outpatient, intensive outpatient or ambulatory withdrawal management is the right starting point. If it is none of them, we say so.

If you or someone you are with is in immediate danger, call or text 988 (the Suicide and Crisis Lifeline) or call 911. Shift Support Network is an outpatient program and is not an emergency service.
