Virtual intensive outpatient

Virtual IOP: how a virtual intensive outpatient program works

A virtual IOP is a structured treatment program for substance use or mental health, delivered by live video. Adults attend about 9 or more hours of group and individual therapy a week, usually for 8 to 12 weeks, and keep working and living at home.

What it isIntensive outpatient care (ASAM Level 2.1) delivered by secure video instead of in a clinic
Hours9 or more hours a week for adults, in 3 to 5 sessions
LengthUsually 8 to 12 weeks, set by clinical progress, not a fixed calendar
FormatGroup therapy, individual therapy, and psychiatry or medication management when needed
Who it fitsAdults who need more than weekly therapy but not 24-hour care
Who it does not fitAnyone needing medical detox, residential care, or crisis stabilization
CostDepends on the plan. Many commercial PPO plans cover IOP in or out of network

What a week looks like

Most of the week is group therapy: 6 to 10 people and one licensed clinician on video, working on skills, relapse prevention, mood, and the situations that set use or symptoms off. Each person also gets an individual session with their own clinician. A prescriber joins when medication is part of the plan.

At Shift Support Network the tracks are fixed and published, so you can plan work around them:

  • Evening: Monday, Tuesday and Thursday, 5:30 to 8:30 PM Pacific
  • Morning: Monday, Tuesday and Friday, 10:00 AM to 1:00 PM Pacific
  • Every track: 9 group hours a week plus one individual session

Full times are on the virtual IOP schedule.

Does virtual IOP work? What the research says

Virtual and in-person IOP use the same clinical methods: cognitive behavioral therapy, relapse prevention, motivational interviewing, and medication where indicated. What changes is the room. Three published studies compare the two directly:

  • Substance use, 3 months after discharge. Among 1,060 IOP patients at a large US treatment center, continuous abstinence, quality of life, psychological well-being and confidence in staying sober did not differ between virtual, hybrid and in-person care. The authors call virtual care "a feasible alternative to in-person-only programming." Gliske K, Welsh JW, Braughton JE, Waller LA, Ngo QM. JMIR Mental Health 2022;9(3):e36263. doi:10.2196/36263
  • Dual diagnosis, symptoms. In a DBT-based dual diagnosis IOP, depression, anxiety and stress fell sharply in both the video and in-person groups, with no significant difference between them. Bean CAL, Aurora P, Maddox CJ, Mekota R, Updegraff A. Journal of Clinical Psychology 2022;78(11):2073-2086. doi:10.1002/jclp.23374
  • Access. Of 3,642 IOP patients in the same center's sample, the virtual program drew patients from 46 states plus Washington, D.C., against 37 states in person. Ngo QM et al. JMIR Formative Research 2022;6(4):e34408. doi:10.2196/34408

None of these studies was randomized, the 3-month survey reached 29% of patients, and the dual diagnosis sample was small (69 people), so the honest summary is "similar results so far," not "better." Virtual care is not the right fit for someone who cannot find a private space at home or who needs in-person medical monitoring. How we track progress is on how we measure IOP outcomes.

Who virtual IOP is for

  • Weekly therapy is not enough, and symptoms or use keep getting worse
  • You are stepping down from detox, residential, or a hospital stay
  • You cannot take 30 days away from a job, children, or school
  • You have substance use and a mental health condition at the same time (dual diagnosis)

A short clinical screen decides the level of care. The criteria are on do I qualify for IOP.

How to check a virtual IOP before you enroll

Telehealth rules follow the patient: the program and its clinicians must be licensed in the state where you are sitting during sessions, not just where the company is based. Check these six things on any program, ours included.

  1. State license or certification. Ask for the number and look it up with your state's behavioral health agency. In California that is the DHCS Licensing and Certification Division.
  2. Clinician licenses. Each therapist should hold a license in your state. California licenses are searchable at search.dca.ca.gov.
  3. Accreditation. Joint Commission accreditation is searchable at qualitycheck.org; CARF accreditation at carf.org.
  4. Level of care in writing. The program should name its ASAM level and weekly hours, not just "intensive".
  5. Published schedule. Real group times, so you can see whether it fits your week.
  6. Cost before you start. A benefit check that gives you a number before the first session.

What virtual IOP costs

Cost depends on the plan, the deductible, and whether the program is in or out of network. Mental health parity law requires most plans to cover IOP on terms comparable to medical care. Ranges and examples are on how much IOP costs, and plan-by-plan detail is under coverage.

Common questions

Virtual IOP questions

What does IOP stand for?

Intensive outpatient program. It is a level of care between weekly therapy and residential treatment, usually 9 or more hours a week for adults.

Is virtual IOP as effective as in-person IOP?

Published comparisons so far show similar results. A 2022 study of 1,060 substance use IOP patients found no difference in 3-month abstinence between virtual and in-person care, and a 2022 dual diagnosis study found the same symptom improvement in both. Neither was randomized. Virtual IOP is not the right fit for someone who needs in-person medical monitoring.

How many hours a week is virtual IOP?

Usually 9 to 15 hours of group and individual therapy a week for adults. Shift's tracks are 9 group hours a week plus one individual session.

How long does virtual IOP last?

Most people attend for 8 to 12 weeks. Length depends on clinical progress, and many programs step patients down to weekly outpatient care at the end.

Can I work while doing virtual IOP?

Yes, on most schedules. Evening and morning tracks exist so people can keep a full-time job. Some people also use FMLA for part of the program.

Does insurance cover virtual IOP?

Many commercial PPO plans cover IOP, including telehealth IOP, in or out of network. A benefit check tells you what your plan pays before you start.

Can I do a virtual IOP from another state?

Only with a program and clinicians licensed in the state where you are located during sessions. Shift's substance use program serves adults located in California; its mental health program also serves adults in New York and New Jersey.

Find out if virtual IOP fits, and what it costs.

A free benefit check and a short clinical screen. You get the level of care, the schedule and the number before you agree to anything.

If you or someone you are with is in immediate danger, call or text 988 (the Suicide and Crisis Lifeline) or call 911. Shift Support Network is an outpatient program and is not an emergency service.
