For hospitals and treatment programs

A step down that is not a gap.

For emergency departments, inpatient psychiatric units, withdrawal management and residential discharge planners in California. The step down starts before the discharge goes cold.

You send the referral. We carry the benefit check, the assessment, the authorization and the updates back to you.

  • Next-business-day intake on most cases
  • We take the discharge summary and work from it
  • Out-of-network benefit check before anyone commits
  • ASAM Criteria 4th edition for every level-of-care decision
  • AM, PM and evening cohorts, 100 percent telehealth
Triage

Who to send to Shift.

Mental health

Lower level of care

Weekly therapy holds it

Symptoms respond to a weekly appointment, and work, safety and home life are intact.

Shift virtual IOP

Three to five hours a day, from home

An adult in California, stable enough to be treated at home, who needs more than one hour a week:

  • Symptoms a weekly appointment is no longer holding
  • Stepping down from inpatient or a partial hospitalization program
  • Anxiety, depression or trauma alongside a substance use concern
  • Cannot step out of work, school or caregiving for weeks
Higher level of care

Not us

Acute risk of harm to self or others, psychiatric instability, or anyone who needs twenty-four hour supervision.

Substance use

Lower level of care

Outpatient is enough

Occasional use that is not affecting work, safety or home life.

Shift virtual IOP

Structured care without a building

An adult in California with a private space to be treated from:

  • Use that is affecting daily life
  • Transitioning from withdrawal management or residential care back home
  • A substance use concern alongside a mental health condition
  • A job, a family or a court schedule that a thirty-day absence would break
Higher level of care

Not us

Medically managed withdrawal, twenty-four hour supervision for physical dependence, or medical instability.

If someone is in crisis, call or text 988, the Suicide and Crisis Lifeline. Call 911 if there is immediate danger. Shift is an outpatient program and is not an emergency service.
What comes back to you

What we handle.

Intake on your discharge timeline
Most referrals start the next business day. Tell us the discharge date and we work back from it.
Benefit check
We read the plan out of network and give the patient a number before the first appointment.
Clinical assessment
A licensed clinician completes the assessment and documents the level-of-care decision on the ASAM Criteria.
Authorization
We submit the authorization and handle medical-necessity review, so your case manager does not.
Closed-loop updates
With the patient's written consent, we confirm attendance and progress back to the referring team. Substance use records move only under 42 CFR Part 2 consent.
An honest no
If the patient needs a higher level than outpatient, we say so on the first call rather than taking the referral.
No payment, in either direction. We do not pay for referrals and we do not accept payment for them. No fee, no commission, no gift, no marketing stipend, no space rental, no free staffing. That is 18 U.S.C. 220 and California Health and Safety Code 11831.6, and it is also how we would run this anyway.

Send a referral.

Two minutes on the referrer form, or call and we take it over the phone. A clinician calls you back the same business day.

Verify your benefits, same business day.

Send us the carrier on your card and we tell you what the plan actually covers before you commit. Call anytime and leave a message. We return calls the same business day.
