It covers what we collect on shiftsupportnetwork.com from people who are not yet patients. Once you become a patient, additional protections apply under federal and state law, and those are documented separately in our Notice of Privacy Practices.
What you give us, what your browser gives us.
What you give us
- Information you enter on an intake or referral form (name, email, phone, insurance, schedule preference, the brief reason for reaching out).
- Messages you send to admin@shiftsupportnetwork.com or other published email addresses.
- Voicemails or calls to our published phone line.
What your browser gives us automatically
- Standard web logs: IP address, browser type, pages viewed, referring URL, time of visit.
- Cookies for session continuity and basic analytics. We do not use cookies to identify you across third-party sites.
- Conversion pixels from Google Analytics 4 and Meta (when configured) for advertising attribution. These can be opted out via your browser settings and ad-blocker.
The honest list.
- Follow up with you about the inquiry you submitted, by the channels you indicated.
- Operate, secure, and improve the website.
- Measure advertising performance in aggregate, never by identifying you to advertisers.
- Respond to legal process (subpoena, court order, regulator request) where required by law.
We do not sell your information. We do not rent your information. We do not share your information with marketing partners.
When and how we contact you.
By submitting an intake form, you consent to receive a follow-up call or email from our admissions team. If you provide a phone number and check the SMS-consent box, you consent to receive text messages (SMS/MMS) from Vanguard Labs LLC about scheduling, intake, and care coordination. Message frequency varies with your inquiry and care activity. Standard message and data rates may apply. Reply STOP to opt out at any time, or HELP for help.
No mobile information (including your phone number and SMS opt-in or consent) will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent are never shared with any third parties. We use your phone number only for the inquiry and care coordination you request.
What you can ask us to do.
- Ask what information we have about you.
- Ask us to correct information that is inaccurate.
- Ask us to delete information that we are not required to retain by law.
- Ask us to stop contacting you (we will, immediately).
- If you are a California resident, additional rights apply under the California Consumer Privacy Act (CCPA) and the California Privacy Rights Act (CPRA). To exercise those, email admin@shiftsupportnetwork.com with the subject line "Privacy Request."
How we protect what we have.
Website submissions are transmitted over TLS and stored in systems with industry-standard access controls. We retain inquiry data for the time needed to follow up on care and to satisfy regulatory record-keeping obligations, typically 2 years for inquiry-only records and longer for active patient records. Patient records are retained under California Department of Health Care Services rules and 42 CFR Part 2 requirements.
Under 18.
This site is intended for adults. We do not knowingly collect information from anyone under the age of 18. If a parent or guardian becomes aware that a child has submitted information, please contact us at admin@shiftsupportnetwork.com and we will delete it.
When we update.
We may update this notice from time to time. The current version is dated below. Material changes will be noted on this page and, where appropriate, communicated to patients separately.
